Sorry for the confusion, but you need both. In order for the image to have rounded edges like the example shown, you need to put in the work skin as the background. However, because the work skin won't work in downloaded works or when AO3 styling is turned off, the pic won't show. That's why you also need to have the image in the work text as well. Does that make sense?
And yes, it does mean you need to make one for each image.
