It happens when Zuko disagrees with and questions someone he thought he could trust. I hadn't been able to articulate it properly before. Except this time the consequence is the dethroning and Iroh's subsequent power over Zuko plus Iroh's changes to the end-of-the-war politics, not Zuko's face getting lit on fire.
