Sanji's about to turn away and melt into the crowd, forgetting this interaction entirely, when the shitty swordsman says something he simply cannot ignore.
“Bet?”Sanji and Zoro hook up once, but it's just to prove a point. The second time is to even the score. The third time, well...and the fourth...and fifth...and maybe Sanji has a problem.

Summary
There’s no exact science to it, but the flowers don’t start climbing up until the later stages. He’s had vines in his lungs for years. If he found out about it earlier, he’d probably have taken the surgery, probably cursed out the stupid cook and blamed him for the way his sullied lungs have been holding him back.
Now though, he isn’t so naive. Even without the excuse of something interfering with his respiration, he knows he simply wasn’t strong enough back then.
Zoro thinks of Sabaody, Thriller Bark, Enies Lobby; of the way a stupid, selfless idiot stood on a broken leg twice just to save his life, and how fucking scared he'd been at the thought of losing the foolish cook. He remembers dozens of other moments like that, and the far fewer, but closely held, instances of quiet affinity they’ve shared outside of a fight. Zoro thinks about how stubborn and single-minded he is in every other field of his life, and knows no surgery would ever stop this.Bookmarked by LegendHasIt

Zoro wakes up in the infirmary unable to move. He can't even blink on his own, let alone speak. And so he's left to listen to his crew as they sit by his bedside, waiting for him to come back to them. He's frustrated and feels utterly useless. He can't even remember how he got injured in the first place.
But he can't be mad at himself for too long, because Sanji comes by and speaks softly to him one evening. And then another. And then another. And Zoro realizes that that *something* he's been imagining between them might be more real than he thought. Now, if only he could move long enough to tell Cook that.

The last thing Zoro expected was for Sanji to be so pleased and to find himself turning red and hot with nerves.

Zoro buys Sanji a gift. Things go a little sideways when Sanji appreciates the gag gift instead of realizing it’s a meant to be a jab, but that’s okay because Zoro has always taken the scenic route to his end destination.His end destination, however, doesn’t seem to be Sanji’s ire after all.
