When Sam tries to kill Crowley he fails and suffers for it. Sam soon becomes Crowley's new favourite plaything.
When Dean finds out what the King has been doing to his baby brother he wants to kill him but he knows better than to underestimate Crowley. Instead they strike a tentative deal. Sam is left alone, Crowley is never to even speak to him, and Crowley gets the Winchester he really wants. The Winchester who could may even come to care for him one day.
As long as he keeps his end of the deal.

Eleven hours with the sole objective of hiding away in this lost corner of Alabama.
No, it wasn't a planned destination from the start—but to Dean, it felt far enough from the Northern Indiana General Hospital, so it was more than fine. -

Dean is one of those people who always fills his coffee cup to the brim. He has the steady hand of a surgeon, and the coffee doesn’t even ripple as he lifts the cup, but he still brings it to his lips quickly, slurping off the excess.
The way he drinks the rest depends on the situation: today, he gulped it all down in one go, and Sam knows that’s not a good sign.
“How long did I sleep?”
“Not even an hour.” Judging by the grimace, he definitely burned his tongue.
Sam turns his head on the pillow, still damp with sweat, and sighs. He doesn’t want to comment on his brother’s hand on his forehead—he already knows why it’s there. He already knows what he would see on his face if he stopped staring at Lucifer, perched on the nightstand like an old owl, smiling and shrugging. “Ah, don’t look at me. I’m not involved. I’ve been quiet the whole time.”
“We need to go to the hospital, Sammy.”

Summary
So I decided to rewrite Sam's experience in the cage from season 11. I added a little bit of spice.
I also suck in writing summaries. I promise my fanfic is better than it's summary.
Instead of Dean and Castiel showing up early to save Sam from Lucifer, they show up later. After Sam gets beaten up, Lucifer decides that he isn't done with Sam yet. So he makes Sam experience far more things. Memories from his time in the cage before plague his mind. Eventually the boys are able to get out and go back to bunker, where Dean takes care of Sam. He's going all Mother Hen on Sam, but Sam couldnt care less, happy to just have his brother by his side.
