Dean has an absolutely horrible relationship with his grandfather. From his racist comments because he'd had kids with a black woman to the even worse homophobic ones because his longtime partner is a man. To say he hates the man is an understatement, but when his mother calls and nags at him to go visit the jerk, he begrudgingly agrees. Cas goes with and they find themselves standing in an empty nursing home with blood on the floor and something growling and scratching from behind the closed door to the common area. What the hell happened, they have no idea, but when Samuel appears, apparently the only person not affected, they decide to take him with. It'll be a nightmare as awful as what's going on outside but Cas is done with the man's abuse. Samuel is going to shut the hell up and be appreciative for once in his damn life, or they just might leave him on the side of the road!
