Sapnap can tell something wasn't right. It's either Dream is stressed out or he did something to piss his older friend off and what it was, he doesn't know. But what he does know is that Dream has been distant. Coming up with elaborate lies to not be around him for too long. He even stops cuddling with him and Sapnap loves cuddles! something is clearly wrong.

“I really don’t understand the point of this…” He said, scrunching up his face as he moved to unwrap the wires from around his head. The room around them was a mess with colorful ornaments and strings of lights scattered on the floor, the tree standing bare in the corner of the room that left him wondering why he was the one being decorated.

Dream’s never been certain of a lot of things in his miserable life, but one thing he can be certain of is that he doesn’t need anybody and nobody needs him. Family means nothing, and everybody is always going to look at him like pathetic scum, especially his parents. Not that they’re wrong.
He honestly wasn’t sure what he was expecting. Of course they would want rid of him once they found out his biggest secret. It was just, of course it had to be around Christmas. Where tons of families are being with and loving each other. Fuck families. Nobody was ever going to love the worthless human being he was. And that’s fine. He doesn’t need them either.

He hadn't been there when they'd found him, Sapnap and Karl, out in the ravine. He didn't need to see it to believe them.
He hadn't needed to hear Tommy whining and screaming and begging, not when he could see Tommy struggling through every breath like a mountain rested on his chest.
It's safe to say, then, that George prefers awake and talking Tommy to the comatose Tommy barely a step from his deathbed.
It's not by much though. At least comatose Tommy hadn't kept pulling his stitches and ripping out feathers in his desperate attempts to get back to Dream.

"George-" Dream starts to say, a frown on his face as he holds George a little tighter, but he doesn’t get a chance to even properly start his reassurances as George interrupts him.
"Why do you even want me here?” George desperately questions, his voice laced with emotion that he’s trying hard to push away, tears threatening to slip down his cheeks. “Why are you still here? How can you even touch me right now? I'm so-" George cuts himself off with a frustrated sound, staring hatefully at himself in the mirror.
With a smile that’s slightly forced, Dream picks up where George left off by asking, "So gorgeous?"
"Don't mock me," George bites out as he pulls away from Dream’s embrace, crossing his arms over his chest protectively and breathing heavily as he tries his best to stay calm.
Feeling like his heart might shatter into thousands of fractured little pieces, Dream gently takes one of George’s hands in his own as he sadly says with nothing but sincerity, "Oh sweetheart, I'm not mocking you. You're stunning. I wish you could see yourself through my eyes."
